FXCRCThe National Fragile X Foundation (NFXF) and the Fragile X Clinical & Research Consortium (FXCRC) are pleased to share the new online Clinic Visit Evaluation with those who have visited one of the clinics in the consortium. The evaluation was designed by the FXCRC’s Executive Council, which includes parents, NFXF staff, clinicians at the clinics and researchers. The anonymous survey will help the NFXF improve overall consortium and clinic services by providing valuable feedback to the consortium as a whole, along with specific feedback addressing the services at individual clinics.

Clinic visitors will be asked to use the form to tell the NFXF how their appointments went—good, bad, or a mixture of both. Individual clinic identities will be noted on each evaluation form, but no personal identifying information of respondents will be asked for or included on the forms. The evaluation process is part of the NFXF’s and FXCRC’s ongoing efforts to make clinic experiences positive, helpful and productive for visitors. Forthright feedback from them will help the clinics meet their goal of continuous service improvement.

Conceived of and co-founded by the NFXF in 2006, the FXCRC now consists of 32 clinics from coast to coast. Clinics strive to provide comprehensive evaluations and treatment recommendations encompassing the latest consensus and evidence-based knowledge from experts in the field of Fragile X-associated Disorders.
